NBI CLEARANCE
Guidelines in Applying for National Bureau of Investigation (NBI) Clearance for Applicants Abroad
Applicants who are living/working abroad must secure and accomplish the NBI Fingerprint Card Form No. 5, available at the Philippine Consulate General, and send it to the NBI in the Philippines for the issuance of an NBI Clearance Certificate. This process also applies to Foreign nationals who had lived in the Philippines and will be required to submit NBI Clearance Certificate.
Below is the procedure for applying for NBI clearance:
1. Obtain an NBI Form No. 5 from Houston PCG, available through the following means:
- Personally obtaining one in Houston PCG on a walk-in basis, no appointment necessary, from 9AM to 3PM, Monday to Friday, except holidays; or
- Requesting one through mail by sending a return envelope to Houston PCG, 9990 Richmond Avenue, Ste. 100N, Houston TX 77042, together with a cover letter requesting for an NBI Form No. 5.
2. Submit the NBI Form No. 5 to the local police station for finger printing.
In Houston, this may be submitted to the Houston Police Department’s Identity Verification Unit, whose details are as follows:

3. After completing the fingerprint impression, submit the original NBI Form No. 5 to Houston PCG personally together with the following requirements:
- Copy of valid passport (data page only).
- Copy of residence card or working visa
- One (1) 2x2 Colored ID Photo with White Background without eyeglasses or colored contact lens taken within six (6) months from application.
- A self stamped addressed envelope (SASE) with tracking number.
Processing Fee is $25.00 by cash.

Submission to the NBI
Applicants can submit their accomplished form and payment to the NBI by two (2) means:
By Mail
- Enclose a money order or bank draft in the amount of two hundred pesos (Php 200.00), in Philippine currency, negotiable in the Philippines, and made payable to the “NBI Director, Taft Avenue, Ermita 1000 Manila, Philippines”. Cash is also accepted.
The clearance fee is one hundred fifteen pesos (Php 115.00) and the remaining eighty-five pesos (Php 85.00) will be used to send the Clearance Certificate through the Philippine Post.
2. Send the accomplished form to:

Through a Representative
If the form will be submitted by the applicant’s representative – Send the accomplished form and payment to your representative, along with a Notarized Letter of Authorization authorizing him/her to transact on your behalf.
