PETITION FOR INCLUSION OF DEPENDENT/S UNDER RA 9225

Any applicant who had previously reacquired or retained Philippine citizenship under R.A. 9225 (dual citizen) from any Philippine Foreign Service Post or office of the Bureau of Immigration, and has failed to include dependent children in the application may file a petition for inclusion of dependent children who are still under 18 years of age and unmarried.

I. Requirements: 

  1. Duly accomplishedPetition for Inclusion of Dependents Application Form (click to download) – form must be notarized, if sent by mail.
  2. One (1) Original and one (1) photocopy of Petitioner’s Dual Citizenship papers (Oath of Allegiance, Identification Certificate, & Approval Order)
  3. Three (3) pieces of dependent child’s 2″ x 2″ photograph with white background taken not more than 3 months before the date of application.
  4. One (1) original and one (1) photocopy of dependent child’s birth certificate.
  5. One (1) original and one (1) photocopy of dependent child’s valid foreign passport bio-page.

The Consular Officer reserves the right to require additional documents from the informant or applicant when necessary.

II. Submission of requirements:

Submission of requirements may be done either by mailing the application together with other requirements to the Philippine Consulate General in Houston at 9990 Richmond Ave. Suite 100N Houston, TX 77042 or by submitting the requirements personally at the Philippine Consulate General in Houston on a walk-in basis, without need of any prior appointment.  

III. Processing Fee:

Processing fee for the petition is $25.00. Fees are non-refundable and should be made payable to the “Philippine Consulate General” in Money Order if the application is sent by mail, or CASH if applied in person.

IV. Releasing of Identification Certificate and Order of Approval

By Pick-up

After three (3) working days, submit the official receipt during pick-up.

In case you will not be available to pick up the document in person, a representative must submit the official receipt, an authorization letter with your signature, and valid IDs

By Mail:

Processed documents will be mailed back to the applicant after three (3) working days upon receipt of the application, provided that the applicant submits a SELF-STAMPED RETURN ENVELOPE with appropriate stamps and tracking number (USPS Express or Priority Mail) or a self-addressed mailing envelope from a courier of choice labelled with the recipient’s address.
